CARPREG I Score: Explanation and Clinical Context The CARPREG I Score (Canadian Cardiac Disease in Pregnancy Risk Score) is a validated tool to estimate the risk of maternal cardiac complications during pregnancy. It evaluates four predictors: prior cardiac event or arrhythmia, NYHA class III-IV or cyanosis, left heart obstruction (including mitral/aortic stenosis or coarctation), and reduced systemic ventricular function (EF <40%). Each predictor scores 1 point, and the total score stratifies patients into low, moderate, or high risk of cardiac complications.

Risk prediction based on original publication: 0 points = 5%, 1 point = 27%, > 2 points = 75% risk of maternal cardiac events. This tool helps clinicians in counseling, risk stratification, and planning multidisciplinary care for pregnant women with heart disease.
