CPET Composite Risk Index for HFrEF & HFpEF: Explanation and Clinical Context The CPET Composite Risk Index integrates key cardiopulmonary exercise testing (CPET) variables to stratify risk in patients with heart failure, both HFrEF and HFpEF. The index combines peak oxygen consumption (Peak VO₂), ventilatory efficiency (VE/VCO₂ slope), heart rate recovery at 1 minute (HRR), and the VE/MVV ratio. Peak VO₂ reflects overall aerobic capacity and cardiac output.

VE/VCO₂ slope assesses ventilatory efficiency and correlates with pulmonary vascular abnormalities and prognosis. HRR indicates autonomic function, while VE/MVV ratio evaluates ventilatory reserve. Each abnormal parameter scores 1 point; total score ranges from 0–4, with 0 indicating low risk, 1–2 intermediate risk, and 3–4 high risk.

This index allows clinicians to integrate multiple CPET parameters for precise prognostic stratification and informs therapeutic decision-making, such as advanced therapies or transplant evaluation.
