Derived Neutrophil-to-Lymphocyte Ratio (dNLR): Explanation and Clinical Context dNLR is calculated as the ratio of neutrophil count to the difference between total white blood cell (WBC) count and neutrophil count (dNLR = Neutrophils / [WBC - Neutrophils]). It is a simple, readily available inflammatory marker derived from routine blood counts. Elevated dNLR has been associated with adverse outcomes in patients with cardiovascular disease, infections, and various malignancies.

It is often used in prognostic models and risk stratification, providing an alternative to the traditional neutrophil-to-lymphocyte ratio (NLR) when lymphocyte counts are not directly measured.
