Exercise Oscillatory Ventilation (EOV) Index: Explanation and Clinical Context Exercise Oscillatory Ventilation (EOV) is characterized by cyclic fluctuations in ventilation during exercise, observed on cardiopulmonary exercise testing (CPET). The EOV Index combines the severity of oscillations, exercise capacity (peak VO2), and ventilatory efficiency (VE/VCO2 slope) to quantify the prognostic burden in heart failure patients. Oscillatory ventilation is an independent predictor of mortality and adverse outcomes in patients with chronic heart failure.

Higher EOV Index values correlate with increased risk and worse prognosis. Clinically, EOV Index can be used for risk stratification, guiding therapy optimization, and assessing candidacy for advanced interventions such as cardiac transplantation or mechanical circulatory support.
