FIB-4 in Cardiac Cirrhosis: Explanation and Clinical Context FIB-4 is a non-invasive scoring system originally validated for assessing liver fibrosis in patients with chronic liver disease and has been adapted for cardiac cirrhosis to estimate fibrosis severity. It uses age, AST (aspartate aminotransferase), ALT (alanine aminotransferase), and platelet count as variables. Lower scores (<1.45) indicate low risk of advanced fibrosis, intermediate scores (1.45–3.25) indicate indeterminate risk, and higher scores (>3.25) suggest high risk of advanced fibrosis.

FIB-4 helps clinicians identify patients at risk of significant liver fibrosis who may benefit from further imaging, monitoring, or interventions, especially in the context of chronic heart failure or cardiac cirrhosis.
