Global Longitudinal Strain Early Change Index (GLS-ECI): Explanation and Clinical Context The Global Longitudinal Strain Early Change Index (GLS-ECI) is a sensitive echocardiographic measure used to detect early myocardial dysfunction before overt changes in left ventricular ejection fraction occur. It is calculated as the percentage change between baseline GLS and follow-up GLS: GLS-ECI (%) = (Follow-up GLS − Baseline GLS) / |Baseline GLS| × 100. A higher negative change (larger absolute reduction in GLS) indicates early subclinical myocardial injury, commonly used in patients undergoing cardiotoxic chemotherapy or at risk of heart failure.

GLS-ECI allows clinicians to detect myocardial dysfunction early and implement cardioprotective strategies promptly.
