HAS-BLED Score: Explanation and Clinical Context HAS-BLED is a validated scoring system designed to assess the 1-year risk of major bleeding in patients with atrial fibrillation who are receiving anticoagulation therapy. It incorporates clinical risk factors including hypertension, abnormal renal or liver function, history of stroke or major bleeding, labile INR, age, concomitant use of drugs that increase bleeding risk, and alcohol use. A total score of ≥3 indicates high bleeding risk, prompting clinicians to carefully monitor the patient, optimize modifiable risk factors, and consider the safest anticoagulation strategy.
Scores of 0–1 indicate low risk, while a score of 2 reflects moderate risk. The HAS-BLED score is intended to guide clinical decision-making and improve patient safety, but it should not be used to withhold anticoagulation when indicated.
