IABP-SHOCK II Risk Score: Explanation and Clinical Context The IABP-SHOCK II Risk Score was developed to provide a simple and rapid risk stratification tool for 30-day mortality in patients with acute myocardial infarction complicated by cardiogenic shock undergoing early PCI. The six independent predictors included: age > 73 years, prior stroke, admission glucose > 191 mg/dL, creatinine > 1.5 mg/dL, arterial lactate > 5 mmol/L and post-PCI TIMI flow grade <3.Points (1 or 2) were attributed per variable, yielding a score from 0 to 9 and stratifying into low (0–2), intermediate (3–4) and high (5–9) risk categories. Clinically, this tool aids in identifying patients at higher risk of early mortality, potentially influencing decisions on more aggressive mechanical circulatory support, intensive monitoring, or referral to specialized centres.

However, limitations apply: the derivation was in AMI-CS patients treated with IABP, and subsequent studies show moderate discrimination in broader cardiogenic shock populations.
