It quantifies the change in predicted probabilities for events and non-events, measuring how much better a new model separates patients who experience an event from those who do not. The IDI is particularly useful in cardiovascular risk prediction, model comparison, and assessing incremental value of novel biomarkers or imaging findings.

In practice, a positive IDI indicates improved discrimination, while a negative IDI suggests worse discrimination.
