Left Atrial Volume Index (LAVI): Explanation and Clinical Context LAVI represents the left atrial volume indexed to body surface area and serves as a reliable marker of chronic diastolic burden. Enlargement of the left atrium occurs gradually as a consequence of prolonged elevation of left ventricular filling pressure. Because of this, LAVI is considered a structural biomarker that reflects chronic diastolic dysfunction more accurately than single Doppler measurements.

The biplane area length method combines planimetered areas from apical four chamber and apical two chamber views with the long axis length of the atrium. This method is recommended because it provides an accurate geometric reconstruction of true atrial volume. Normal values based on population studies indicate that a LAVI of up to thirty four ml per square meter is considered normal.

Values above this threshold suggest left atrial enlargement. Mild enlargement ranges from thirty five to forty one ml per square meter. Moderate enlargement is typically defined as forty two to forty eight ml per square meter.

Severe enlargement is characterized by values exceeding forty eight ml per square meter. Clinical implications of elevated LAVI include a strong association with diastolic dysfunction, increased risk of atrial fibrillation, higher morbidity in heart failure patients, and worse prognosis following myocardial infarction. In routine practice, LAVI is useful for risk stratification, assessment of chronicity of elevated filling pressures, and evaluation of therapeutic response in heart failure management.
