Lactate Clearance Index: Explanation and Clinical Context Lactate Clearance Index represents the rate at which elevated blood lactate levels decline over time following resuscitation efforts. It reflects the balance between tissue oxygen delivery and utilization, and thus serves as a dynamic marker of global tissue perfusion. A higher clearance indicates effective resuscitation and improved prognosis, while persistently elevated lactate levels suggest ongoing hypoperfusion and metabolic dysfunction.
Definition and Formula: Lactate Clearance (%) = [(Initial Lactate − Repeat Lactate) / Initial Lactate] × 100 Normal and Clinical Values: A lactate clearance >10% within the first 6 hours of resuscitation is generally associated with a favorable outcome, while a clearance <10% indicates inadequate response to therapy and higher mortality risk. Clinical Significance: Lactate clearance serves as a prognostic tool in the management of sepsis, septic shock, and cardiogenic shock. It can guide early goal-directed therapy, help titrate fluid resuscitation and vasoactive agents, and assess tissue perfusion status noninvasively.
Monitoring clearance over time is more informative than a single lactate value because it reflects both the production and elimination dynamics of lactate under stress conditions.
