ACS-MCS (Mechanical Circulatory Support) Score: Explanation and Clinical Context The ACS-MCS score is a simple, four-point risk score developed to predict 30-day all-cause mortality in patients with cardiogenic shock due to acute coronary syndrome who require short-term mechanical circulatory support (ST-MCS). The model assigns 1 point each for: age > 60 years, pre-ST-MCS lactate > 2.5 mmol/L, presence of acute kidney injury (AKI) at the time of ST-MCS implementation, and SCAI cardiogenic shock stage E (extremis). Total score ranges 0–4 and stratifies patients into low (0–1), intermediate (2), and high (3–4) risk groups.

The score was derived and internally validated using the UCAR Shock database (n=159), demonstrated good calibration and discrimination (AUC ≈ 0.80), and may assist clinicians in early risk stratification and device selection as an adjunct to clinical judgment.
