Medicine Half Life and Clearance Explanation and Clinical Context The concept of half life describes the amount of time required for the concentration of a drug in the body to fall to half of its initial value and it plays a fundamental role in determining dosing interval, steady state time, and drug accumulation especially in long term pharmacotherapy. Clearance represents the volume of plasma completely cleared of a drug per unit of time and it reflects the capacity of the liver and kidney to eliminate substances from systemic circulation. A drug with high clearance will be removed more rapidly leading to a shorter half life while a drug with low clearance tends to stay longer in the body and requires careful dose adjustment.

Half life can be estimated using the pharmacokinetic formula 0 point 693 multiplied by volume of distribution divided by clearance which provides clinicians with a rational and quantitative method to calculate how long a medication persists and how frequently it should be administered. Understanding the relationship between volume of distribution, clearance, and half life allows more accurate titration in patients with renal or hepatic impairment and is essential in drugs with narrow therapeutic index.
