The index is designed to differentiate transient wheezing associated with viral illness from an evolving chronic airway disease pattern. The tool uses a combination of major and minor clinical criteria that reflect genetic predisposition atopy and airway inflammation.

A positive result indicates a significantly increased probability of developing persistent asthma by school age while a negative result suggests a lower likelihood. The Modified API supports early counseling monitoring and identification of children who may benefit from targeted prevention strategies or early therapeutic intervention.
