The Neonatal Sepsis Score is intended to aid clinicians in recognizing early signs of infection by integrating simple bedside clinical indicators. Temperature instability reflects impaired thermoregulation caused by inflammation or infection.

Heart rate abnormalities can represent either compensatory responses or cardiovascular depression. Respiratory distress frequently emerges as an early manifestation of systemic infection due to metabolic acidosis or pulmonary involvement. Poor perfusion indicates circulatory compromise and may point to evolving septic shock.

Feeding difficulty and lethargy reflect altered neurologic and metabolic function during infection. These variables contribute cumulatively to increase clinical suspicion. Although the score supports early detection it does not replace laboratory evaluation or clinical judgment.
