Enter preoperative variables below and click Calculate. NSQIP (Gupta) MICA: Explanation, Clinical Context, and References This calculator implements the ACS NSQIP-derived Gupta model to estimate the probability of myocardial infarction or cardiac arrest within 30 days after surgery.

It uses five predictors: type of surgery, functional status, ASA class, serum creatinine >1.5 mg/dL, and age. The logistic model provides a continuous estimate of cardiac complication risk, validated with C-statistics around 0.88 in the original cohort. Clinical significance: The MICA score supports perioperative risk stratification, preoperative optimization, and informed consent.

It should be interpreted alongside clinical judgment, particularly in patients with extremes of comorbidity or unique surgical risk profiles. Model equation: x = 0.02 × Age + Functional status coefficient + ASA coefficient + Creatinine coefficient + Surgery coefficient − 5.25; Risk = exp(x) / (1 + exp(x)).
