When the treatment reduces adverse outcomes the absolute risk reduction becomes positive and the result is expressed as NNT. If the treatment increases the probability of adverse outcomes the absolute risk reduction becomes negative and the result is expressed as NNH.

This metric helps clinicians translate statistical outcomes into meaningful clinical estimates. Values closer to one indicate stronger clinical effect while very high values suggest limited practical benefit. NNT is commonly used to evaluate medications, acute interventions, preventive therapies, and guideline recommendations.

Interpreting NNT should always consider event rate, baseline risk, trial duration, and clinical significance of the outcome. A single NNT value does not capture the full complexity of patient populations or treatment thresholds. A low NNT is clinically attractive for high risk groups but the same therapy may yield a higher NNT in lower risk groups.
