Oxygen Uptake Efficiency Slope (OUES): Explanation and Clinical Context OUES is a submaximal index derived from cardiopulmonary exercise testing (CPET) that quantifies the efficiency of oxygen uptake relative to ventilation. It is calculated using the linear relationship between oxygen consumption (VO2) and the logarithm of minute ventilation (VE), typically expressed as VO2 = a × log10(VE) + b, where the slope 'a' represents OUES. OUES is considered less dependent on patient effort than peak VO2, making it useful in populations unable to perform maximal exercise, such as heart failure or pulmonary disease patients.

Higher OUES values indicate more efficient oxygen uptake; lower values suggest impaired cardiopulmonary function. OUES has been validated as a prognostic marker in heart failure, predicting mortality and clinical outcomes independently of peak VO2.
