FiO2 Mean Airway Pressure MAP PaO2 Oxygenation Index OI Explanation and Clinical Context Oxygenation Index is a validated measure used to quantify the severity of hypoxic respiratory failure and to evaluate the degree of ventilatory support required to maintain adequate oxygenation. The formula incorporates the fraction of inspired oxygen with mean airway pressure and arterial oxygen tension which allows clinicians to understand the interaction between ventilation and oxygenation. Higher values reflect more severe impairment in gas exchange and increasing difficulty in achieving sufficient oxygen levels despite ventilatory support.

Normal values are generally less than five in individuals without respiratory pathology. A value greater than fifteen suggests significant respiratory compromise. A value above twenty five is associated with severe hypoxemia and is often used as a threshold for considering advanced support including extracorporeal membrane oxygenation in neonatal and pediatric populations.

The index therefore provides a quantitative guide for clinical decision making and helps clinicians track the progression of respiratory failure and evaluate responses to therapeutic interventions.
