Porto-Systemic Flow Ratio (PSFR): Explanation and Clinical Context The Porto-Systemic Flow Ratio (PSFR) quantifies the relative blood flow between the portal vein and the hepatic vein, reflecting the balance of portal and systemic circulation in patients with chronic liver disease and portal hypertension. A PSFR near 1 indicates a normal physiologic balance, whereas deviations may reflect shunting, hepatic congestion, or portal hypertension severity. This tool is clinically useful to estimate the degree of porto-systemic shunting, assess risk of complications such as variceal bleeding or hepatic encephalopathy, and guide management in cirrhosis and advanced liver disease.
