Prognostic Nutritional Index (PNI): Explanation and Clinical Context Prognostic Nutritional Index (PNI) is a clinical tool used to assess the nutritional and immunological status of patients, originally developed by Onodera et al. in 1984. It is calculated using serum albumin concentration and total lymphocyte count. PNI has been validated as a predictor of postoperative complications, surgical outcomes, and prognosis in patients with malignancies and cardiovascular diseases.

The higher the PNI, the better the nutritional and immune status. PNI < 40 generally indicates poor nutritional status and higher risk of adverse outcomes. PNI helps clinicians in risk stratification, perioperative management, and identifying patients who may benefit from nutritional interventions.
