PYP Scintigraphy Visual Score: Explanation and Clinical Context PYP (99mTechnetium-Pyrophosphate) scintigraphy is a non-invasive imaging technique used to detect cardiac amyloidosis, particularly transthyretin amyloidosis (ATTR-CM). The visual score ranges from 0 to 3 based on myocardial radiotracer uptake relative to bone: - Grade 0: No cardiac uptake; ATTR-CM is very unlikely. - Grade 1: Mild uptake, less than bone; ATTR-CM possible but low likelihood. - Grade 2: Moderate uptake, equal to bone; suggests probable ATTR-CM, especially in absence of monoclonal protein. - Grade 3: High uptake, greater than bone; highly suggestive of ATTR-CM. This visual scoring system allows clinicians to stratify patients, guide further diagnostic testing (such as biopsy or genetic testing), and support early therapeutic decisions.
