It uses three fundamental clinical variables Glasgow Coma Scale systolic blood pressure and respiratory rate. Each variable is assigned a coded value based on predefined clinical ranges that reflect the degree of physiological compromise.

The final score is calculated using a weighted formula that gives greater influence to neurologic status followed by perfusion and respiratory parameters. The score ranges from zero to seven point eight four and higher values reflect better physiological stability and improved survival probability. Revised Trauma Score is commonly used in prehospital triage trauma team activation and early decision making in emergency departments.

A low score is associated with high mortality risk and can guide the need for rapid transport to trauma centers advanced resuscitation and early imaging. Its simplicity reproducibility and reliance on readily available bedside measurements make it particularly valuable in high acuity settings including mass casualty events.
