Reynolds Risk Score (RRS): Comprehensive Explanation and Clinical Context The Reynolds Risk Score is a 10-year cardiovascular risk prediction model developed by Ridker PM and colleagues (2007). It estimates the probability of major cardiovascular events (myocardial infarction, ischemic stroke, or coronary revascularization) among individuals without existing cardiovascular disease or diabetes. The RRS combines traditional risk factors (age, systolic BP, total and HDL cholesterol, smoking) with two additional predictors: high-sensitivity C-reactive protein (hs-CRP) and family history of premature myocardial infarction.

Normal Range & Interpretation A 10-year risk <5% is considered low, 5-9% intermediate, and >10% high. Higher scores reflect greater probability of a cardiovascular event within 10 years. Clinical Significance By integrating an inflammatory biomarker (hs-CRP), RRS enhances detection of hidden cardiovascular risk not captured by traditional scores like the Framingham Risk Score, improving individualized prevention strategies.

Clinical Interpretation Summary Use the RRS for adults without diabetes or established CVD. Patients with an estimated 10-year risk >10% may benefit from more intensive preventive interventions, such as statin therapy, blood pressure control, and lifestyle modification.
