Signal-Averaged ECG (SAECG) Abnormality Index: Explanation and Clinical Context SAECG is a specialized electrocardiographic technique used to detect subtle abnormalities in ventricular depolarization that are not visible on standard ECG. The SAECG Abnormality Index quantifies risk for ventricular arrhythmias by scoring three primary parameters: filtered QRS duration, low amplitude signal duration, and root mean square voltage of the terminal 40 ms. Each abnormal parameter contributes one point to the index.

A score of 0 indicates normal ventricular conduction, 1 suggests borderline abnormality, 2 indicates moderate risk, and 3 indicates high risk for ventricular arrhythmias, particularly in post-myocardial infarction patients. This tool helps clinicians identify patients who may benefit from further risk stratification or prophylactic interventions such as implantable cardioverter-defibrillators (ICDs).
