Seattle Heart Failure Model (SHFM): Explanation and Clinical Context The SHFM is a validated multivariable model to estimate mortality risk in patients with chronic heart failure based on clinical, laboratory, and treatment variables. It was developed using data from the PRAISE I cohort and externally validated in multiple populations. The model calculates a risk score (SHFM Score) using regression coefficients applied to patient data.

Lower (more negative) scores indicate higher mortality risk. The SHFM can support prognosis estimation and guide therapeutic planning for heart failure patients.
