Spleen Injury Grading Scale Explanation and Clinical Context The Spleen Injury Grading Scale provides a structured method for classifying splenic trauma using imaging based anatomical criteria. This system helps clinicians understand the severity of parenchymal disruption hematoma size vessel involvement and the extent of devascularization. Grade I reflects minor injury with limited subcapsular hematoma or shallow laceration.

Grade II represents moderate parenchymal disruption or hematoma expansion. Grade III indicates deeper lacerations or larger hematomas that may compromise organ stability. Grade IV involves significant vascular injury resulting in substantial loss of blood flow to splenic tissue.

Grade V represents the most severe pattern including a completely shattered spleen or major hilar injury producing near total devascularization. This tool supports clinical decision making for non operative management angioembolization planning or operative intervention depending on stability and associated injuries.
