It relies on six observable parameters including quality of cry, reaction to parents, state variation, color, hydration, and response to social overtures. Each parameter is scored from one to three and higher total score indicates better clinical appearance.

The tool supports decision making for febrile children by helping clinicians identify those who may be at low, moderate, or high risk for serious bacterial infection. A score of thirteen to eighteen indicates low risk, nine to twelve indicates moderate risk, and below nine suggests high risk and warrants closer observation or further investigation.
