It reflects the integrated risk of insulin resistance, visceral adiposity, and dyslipidemia—three core components of the metabolic syndrome defined by the National Cholesterol Education Program (NCEP) Adult Treatment Panel III (ATP III). This calculator aligns with ATP III and subsequent ACC/AHA and IDF consensus statements that highlight waist circumference and lipid ratios as central to metabolic risk prediction.

A higher CMI indicates a greater likelihood of cardiometabolic disorders including type 2 diabetes, atherosclerosis, and coronary artery disease. In modern risk modeling (Wakabayashi & Daimon, 2015; Liu et al., 2023), CMI has demonstrated stronger predictive value for cardiometabolic syndrome than traditional markers alone. It provides a simple yet powerful clinical metric for identifying high-risk individuals even when fasting glucose or LDL-C levels appear normal.

Interpretation Summary: Low-risk individuals exhibit balanced lipid metabolism and normal waist circumference. Moderate and high-risk categories suggest progressive insulin resistance and visceral fat accumulation, warranting lifestyle intervention, nutritional counseling, and possibly pharmacologic management.
