APACHE II score explanation and clinical context APACHE II is a severity scoring system developed to estimate risk of hospital mortality for patients admitted to intensive care. The score is the sum of an acute physiology score derived from the worst value in the first 24 hours for twelve physiologic variables plus points for age plus points for chronic health status. A higher total score indicates greater physiologic derangement and higher risk of death.

The original method converts APACHE II into a predicted death probability using a logistic regression equation. The original formula uses logit equals negative 3.517 plus 0.146 times the APACHE II score plus an optional diagnostic category weight for certain operative categories. Predicted probability of death equals e to the logit divided by 1 plus e to the logit.

Clinical use requires accurate worst value selection in the first 24 hours and careful interpretation because individual patient prognosis can vary and calibration may differ by local population and case mix.
