ARDS Berlin Criteria Classifier Explanation and Clinical Context The Berlin Criteria provides a standardized method to diagnose acute respiratory distress syndrome based on the degree of hypoxemia, the requirement of positive end expiratory pressure, and the presence of bilateral opacities not fully explained by cardiac failure. The PaO2 FiO2 ratio defines the severity group. A ratio of three hundred or less with mandatory positive end expiratory pressure of five or more indicates mild disease.

A ratio of two hundred or less indicates moderate disease. A ratio of one hundred or less indicates severe disease. This classification reflects the intensity of gas exchange impairment and correlates with mortality risk.

The criteria must be applied in patients with an acute onset of respiratory failure within one week of a known clinical insult and with imaging findings that confirm diffuse involvement. The classifier helps clinicians evaluate disease burden, anticipate prognosis, and guide escalation of respiratory support strategies including consideration of prone positioning and extracorporeal support in advanced cases.
