It incorporates serum PSA concentration the Gleason grading system the clinical T stage classification and the percentage of biopsy cores involved with malignancy. The score ranges from zero to ten and the cumulative value provides an estimation of disease aggressiveness and probability of biochemical recurrence after treatment.

Values from zero to two generally indicate low risk disease. Values from three to five indicate intermediate risk disease. Values of six or higher indicate high risk disease with an increased likelihood of progression and biologically active tumor behavior.

This scoring system assists clinicians in counseling patients at diagnosis guiding decision making for active surveillance surgical therapy radiotherapy or combined treatment strategies.
