Mantle Cell Lymphoma International Prognostic Index and simplified approach explanation The Mantle Cell Lymphoma International Prognostic Index provides a clinical estimate of prognosis based on four baseline variables age ECOG performance status serum lactate dehydrogenase relative to the laboratory upper limit of normal and white blood cell count measured in 10 to the 9 per liter. The original MIPI score is computed as a weighted sum using coefficients derived from Cox regression and yields a continuous score that is commonly categorized with cutoffs to define low intermediate and high risk groups where a value below 5.7 is considered low risk 5.7 to 6.2 intermediate risk and above 6.2 high risk. A simplified MIPI uses integer points assigned to categories of each variable to produce a total points score that is easier to calculate at the bedside or in a web tool The simplified score strata are low risk with total points of 0 to 3 intermediate risk with total points of 4 or 5 and high risk with total points of 6 to 11.

Both approaches are intended for prognostic stratification and not as a substitute for clinical judgment or for making treatment decisions without specialist multidisciplinary input The biological MIPI that incorporates Ki 67 proliferation index or other molecular markers may refine prognostic classification when those data are available.
