Cardiac Index Calculator: Explanation and Clinical Context Cardiac Index represents the cardiac output normalized to body surface area, allowing clinicians to interpret cardiac performance in a manner that accounts for differences in body habitus. This parameter provides a more individualized assessment of circulatory adequacy compared to cardiac output alone. It is expressed in liters per minute per square meter and is widely used in critical care settings, perioperative cardiovascular management, and advanced heart failure evaluation.

A normal Cardiac Index typically ranges from 2.5 to 4.2 liters per minute per square meter. Values below this range suggest impaired forward flow and may indicate cardiogenic shock, advanced decompensated heart failure, or hemodynamic deterioration in conditions such as acute myocardial infarction or severe valvular heart disease. Conversely, elevated values may appear in hyperdynamic states such as sepsis or severe anemia.

Interpretation should always be contextualized with preload, afterload, contractility, and clinical presentation. Cardiac Index is fundamental in guiding management decisions including titration of inotropes, vasopressors, mechanical circulatory support, and fluid optimization. In advanced heart failure, CI helps determine candidacy for therapies such as inotropes, left ventricular assist devices, and heart transplantation.

In shock states, it aids differentiation between cardiogenic and distributive etiologies when combined with systemic vascular resistance and filling pressures.
