It evaluates four observable domains that reflect physiologic and behavioral responses to pain. These include facial expression body movement compliance with the ventilator and muscle tension.

Each domain is scored from zero to two which produces a total score of zero to eight. A higher score suggests a stronger probability that the patient is experiencing significant pain. This tool is especially helpful in patients who are unable to verbalize pain due to mechanical ventilation sedation or neurologic impairment.

It provides a systematic method to identify discomfort guide analgesic therapy and monitor treatment response in a dynamic critical care environment. Clinical interpretation typically considers scores above two as indicating the presence of pain that may require intervention. Regular use of the tool improves patient comfort supports ethical care and reduces the risk of untreated pain.
