Central Venous Pressure (CVP) reflects right atrial pressure, indicating right ventricular preload and venous congestion. Pulmonary Capillary Wedge Pressure (PCWP) reflects left atrial pressure and left ventricular filling status.

The ratio CVP/PCWP helps identify disproportionate right-sided pressure relative to left-sided filling pressure, which has been associated with higher morbidity and mortality. In studies, a CVP/PCWP ratio < 0.63 is generally considered low risk, 0.64–0.85 intermediate risk, and > 0.85 high risk for adverse outcomes. This tool aids clinicians in risk stratification, optimizing hemodynamic management, and anticipating complications in critically ill cardiac patients.
