It incorporates key parameters known to influence cardiovascular event risk (age, systolic blood pressure, resting heart rate, exercise tolerance, and known coronary artery disease) in the diving environment. Immersion, cold-water exposure, hyperbaric stress, and exertion during diving impose increased preload, afterload, sympathetic/parasympathetic challenges, and risk of arrhythmia or ischemia.

In particular, the literature suggests that divers aged ≥50 years or those with cardiovascular risk factors may exceed an estimated event risk threshold of ~2% per year and thus warrant enhanced evaluation.
