Get With The Guidelines–Heart Failure (GWTG-HF) Rehospitalization Risk Score: Explanation and Clinical Context The GWTG-HF Rehospitalization Risk Score was developed from a large national registry to estimate the likelihood of 30-day readmission after discharge for patients hospitalized with heart failure. It integrates key clinical parameters available at admission and discharge, including age, blood pressure, heart rate, serum sodium, serum creatinine, race, and comorbid COPD. This model was derived and validated in over 30,000 patients enrolled in the American Heart Association’s Get With The Guidelines–Heart Failure program and demonstrated good discrimination (C-statistic ≈ 0.63–0.68).

A higher score reflects a higher probability of early readmission, guiding clinicians to optimize transitional care, arrange closer follow-up, and implement multidisciplinary discharge planning. Normal Range and Interpretation: There is no “normal” risk value, but population medians typically correspond to a 30-day readmission probability of about 18%. Patients with risk scores below 10% are often categorized as low risk, whereas those exceeding 20% are considered high risk for early readmission.

Clinical Significance: Early readmission after heart failure hospitalization indicates poor transition of care and suboptimal control of volume status, medication adherence, or social support. Predicting readmission risk allows individualized care planning, early outpatient visits, and targeted interventions such as telemonitoring and nurse-led follow-up.
