ICE Guidance Difficulty Index (ICE-DI): Explanation and Clinical Context The ICE Guidance Difficulty Index (ICE-DI) is a scoring system developed to estimate the procedural difficulty of intracardiac echocardiography (ICE)-guided interventions, particularly during catheter ablation or device implantation procedures. This index integrates several anatomic and clinical factors that can affect acoustic window quality and catheter manipulation stability. It helps operators anticipate technical challenges, optimize procedural planning, and determine the need for adjunctive imaging modalities such as transesophageal echocardiography (TEE).

Key determinants include body habitus (BMI), left ventricular systolic function (LVEF), left atrial size (LAVI), tricuspid regurgitation severity, right atrial enlargement, and history of cardiac surgery. Higher scores reflect increased procedural complexity, prolonged procedure time, and greater likelihood of requiring alternative imaging support. A total score ≤2 suggests low difficulty with expected smooth ICE-guided visualization.

Scores between 3–4 represent moderate technical demand, while ≥5 indicates high difficulty and potentially limited ICE view of left atrial structures. The ICE-DI score thus supports personalized procedural strategies in electrophysiology and interventional cardiology, promoting patient safety and imaging efficiency.
