It is calculated using fasting plasma glucose and fasting insulin, based on the HOMA-IR model. A lower INSPIRE Index indicates higher insulin sensitivity and lower cardiometabolic risk, whereas higher values suggest insulin resistance and increased risk for diabetes and cardiovascular events.

Clinicians can use this index for early detection, risk stratification, and individualized lifestyle or pharmacologic interventions to prevent disease progression.
