LECOM (Lead Extraction COMplexity) Score: Explanation and Clinical Context The LECOM score is a predictive tool developed to assess the complexity of transvenous lead extraction (TLE) procedures. It incorporates clinical and procedural factors to estimate the likelihood of a difficult extraction. The score is calculated based on the following parameters: - Age at First CIED Implantation: Patients implanted before 51 years are assigned 2 points. - Number of Abandoned Leads: Presence of abandoned leads adds 2 points. - Number of Previous Procedures: More than one prior procedure contributes 2 points. - Passive Fixation Leads: Use of passive fixation leads adds 2 points. - Number of Leads to be Extracted: Extracting multiple leads contributes 2 points. - Dwell Time Ratio: A ratio greater than 0.13 adds 2 points.

The total points are summed, and the probability of a complex extraction is calculated using the formula: Probability (%) = [1 / (1 + 55.34 * 0.754^X)] * 100 where X is the total points. A higher score indicates a greater likelihood of procedural complexity.
