A reduced LVEF < 35% is associated with a significantly higher risk of sudden cardiac death (SCD), making these patients candidates for ICD implantation for primary prevention. Patients with LVEF between 36-40% require individualized assessment, considering additional risk factors such as NYHA functional class, history of ventricular arrhythmias, and myocardial scarring.

Patients with LVEF >40% are generally at lower risk for SCD, and ICD is typically not indicated solely based on LVEF. Optimal medical therapy remains the cornerstone.
