The model is based on large meta-analyses demonstrating that for each 1 mmol/L reduction in LDL-C, there is an approximate 22% relative reduction in major vascular events, including myocardial infarction, stroke, and cardiovascular death. Clinicians can use this calculator to estimate the potential absolute risk reduction (ARR) and number needed to treat (NNT) over 10 years, based on the patient's baseline LDL-C, the expected percentage reduction with therapy (e.g., statins, ezetimibe, PCSK9 inhibitors), and the individual's baseline ASCVD risk.

For instance, a patient with a 10-year ASCVD risk of 20% and a 2 mmol/L LDL-C reduction (≈44% relative risk reduction) would have an ARR of 8.8%, corresponding to an NNT of approximately 11 over 10 years. This model assumes linearity of risk reduction per mmol/L LDL-C decrease and may underestimate or overestimate true benefit in special populations, such as those with diabetes, very high risk, or established coronary artery disease.
