LV Global Strain Abnormality Index (HCM/DCM): Explanation and Clinical Context The LV Global Strain Abnormality Index quantifies the extent of segmental myocardial dysfunction in patients with hypertrophic cardiomyopathy (HCM) or dilated cardiomyopathy (DCM). It combines global longitudinal strain (GLS) assessment and segmental strain abnormalities to provide a percentage score reflecting myocardial involvement. GLS is measured via speckle-tracking echocardiography, with values less negative than -16% considered abnormal in HCM/DCM.

The segmental strain abnormality count is based on a 17-segment LV model. The index is calculated as (Number of abnormal segments / 17) × 100%. Higher LV Global Strain Abnormality Index values are associated with increased risk of adverse outcomes, including heart failure progression, arrhythmias, and sudden cardiac death, providing clinicians with a quantitative tool for risk stratification and monitoring.
