It includes four components which are stool frequency rectal bleeding endoscopic appearance and physician global assessment. Each component is scored from zero to three leading to a total possible score of zero to twelve.

Higher scores reflect more active disease. Normal or inactive disease is usually associated with scores of two or less. Mild disease is suggested by scores of three to five moderate disease corresponds to scores of six to ten and severe disease is typically reflected by scores above ten.

The tool is frequently used in clinical trials and routine practice to evaluate treatment response to determine the need for therapy escalation and to assess disease control over time.
