Bilirubin mg per dL INR Creatinine mg per dL Sodium mEq per L MELD and MELD Na Score Explanation and Clinical Context The Model for End Stage Liver Disease score is an evidence based severity index developed to estimate short term mortality in patients with chronic liver disease. It is calculated from serum bilirubin international normalized ratio and serum creatinine values to represent the degree of hepatic dysfunction and renal impairment. Because serum sodium strongly correlates with prognosis in advanced cirrhosis the MELD Na score was introduced to refine prognostic accuracy by incorporating the effect of hyponatremia.

Higher scores reflect greater mortality risk and guide clinical decisions related to transplant listing prioritization assessment of disease severity and overall treatment strategy. Both MELD and MELD Na are widely used in hepatology to support objective and reproducible evaluation of patients with decompensated liver disease. Reference Kamath PS Wiesner RH Malinchoc M et al.
