Milan Criteria for Liver Transplant in Hepatocellular Carcinoma Explanation and Clinical Context The Milan Criteria define a simple morphological selection rule for liver transplant candidacy in patients with hepatocellular carcinoma. The rule is a single tumor with maximum diameter of 5 centimeters or less or up to three tumor nodules with each nodule 3 centimeters or less in diameter. These size and number rules apply in the absence of macroscopic vascular invasion and in the absence of extrahepatic tumor spread on imaging.

The Milan Criteria were introduced to select patients with early tumor burden who derive a substantial survival benefit from liver transplant and who have an acceptably low risk of tumor recurrence after transplant. The criteria remain widely used as a baseline selection tool and as a comparator for expanded criteria and downstaging strategies in liver transplant programs.
