OPTIMIZE-HF Risk Score: Explanation and Clinical Context OPTIMIZE-HF (Organized Program to Initiate Lifesaving Treatment in Hospitalized Patients with Heart Failure) Score is a validated clinical tool developed to estimate in-hospital mortality risk for patients admitted with acute decompensated heart failure. It uses readily available clinical variables at hospital admission including age, systolic blood pressure, heart rate, serum sodium, serum creatinine, left ventricular ejection fraction (LVEF), and whether heart failure was the primary reason for admission. The total score stratifies patients into low, moderate, or high risk for in-hospital mortality.

This tool assists clinicians in risk stratification, decision-making regarding intensity of monitoring, and allocation of hospital resources.
