Radiation-Induced Heart Disease (RIHD) Dose Index: Explanation and Clinical Context RIHD Dose Index (RIHD-DI) is a predictive measure used to estimate the risk of cardiac complications in patients receiving thoracic radiotherapy. The index incorporates key dosimetric parameters: Mean Heart Dose, Maximum Heart Dose, and the percentage of heart volume receiving >25 Gy (V25). This model is based on epidemiological and clinical studies demonstrating a linear relationship between radiation dose to the heart and subsequent risk of ischemic heart disease and heart failure.
Higher RIHD-DI values indicate a higher likelihood of developing radiation-induced cardiac events and can guide clinicians in optimizing radiotherapy plans to minimize cardiac exposure. Clinical application includes risk stratification, patient counseling, and monitoring strategies for early detection of RIHD.
