Renal Angina Index (RAI): Explanation and Clinical Context The Renal Angina Index (RAI) is a validated tool to predict the development of severe acute kidney injury (AKI) in critically ill patients, especially pediatric ICU patients. It is calculated by multiplying a Risk Score (based on underlying condition severity) with an Injury Score (based on early signs such as fluid overload or rise in creatinine). RAI > 8 indicates high risk, 4–7 moderate risk, and <4 low risk of developing severe AKI within 72 hours.

This tool allows clinicians to stratify patients early, guide monitoring, and implement preventive interventions.
